ANACORTES, WA - The Washington state Department of Labor & Industries has fined an oil refinery where 4 people died as a result of an industrial accident Friday morning . Three other people have been hurt in the accident.

In 2008, a several month investigation turned up 17 serious safety and health violations as part of a national emphasis to inspect petroleum facilities.

Inspectors found 150 instances of deficiencies, including where the company didn't ensure safe work practices for energy control and failed to update safety information when changes were made to technology and equipment.

Last year, Labor & Industries fined the Tesoro Anacortes facility, $87,500. 

L & I spokesman Hector Castillo said the company has appealed.

The fine has since been reduced  to $12,250.
